Conclusions The severe DDS from which these 2 patients were suffering was extremely responsive to STN stimulation. The classically recognized motor improvement was associated with the complete disappearance of the manic psychosis and compulsive dopaminergic addiction. These findings suggest that, although DDS is not yet an indication for STN DBS, it should not be regarded as an obstacle. In fact, it might be a real possibility to improve the condition of these young and severely impaired patients. Further reports will confirm or deny these primary results. In our opinion, a postoperative drug strategy involving early withdrawal might be one of the main components of effective addiction therapy. In addition, the results obtained here suggest that STN DBS might provide a useful tool for studying addiction in animal models and possibly also in humans.
